EMS Incident/Medical Records Request Form

There are four ways to submit a Public Records Request Form. 

1. Fill out and submit the form online. This is the fastest method.

2. Fill out the form online, print it and submit it via postal mail or in person. 

3. Fill out the form online, print it and submit it via fax to the City Clerk's Office at (702) 649-3846.

4. If this is a request from the news media, contact the City of North Las Vegas Communications Department. 

Please note: Payment upon receipt of records and/or a deposit in advance of receipt of records may be required. 

The City of North Las Vegas provides access to public records pursuant to Nevada's Public Records Act (Nevada Revised Statutes Chapter 239). Please note that a request for data or a database is not a public records request and the department is not required to organize data to create a record that does not exist at the time of the request. A request should be for an identifiable record and will be handled as expeditiously as possible. Record(s) that are not readily available or voluminous may require payment for the cost of research and/or redact confidential information.
